The Lakewood City School District has a new online registration and student data system. Instead of Infosnap, we are collecting new and returning student data via Registration Gateway. The new system is accessible anywhere there is a computer with an internet connection, including at home or the public library. There is also a kiosk available at the Board of Education building in the registration office.

If you are new to Lakewood City Schools and are registering for the first time, or if you are returning to Lakewood City Schools and your child(ren) was previously withdrawn, please complete the online New Student Registration form by clicking on the button below:
Note: If you received a username and password from Lakewood City Schools, please use the Back to School gateway link in the section below.
Only legal residents who are the parent or legal guardian of the child may complete the registration process. All certified court orders pertaining to guardianship and custody must be presented at the time of registration. The Lakewood City School District does not grant temporary enrollment (ORC 3313.64 E).

If you are not new to Lakewood City Schools and your child(ren) was enrolled last school year, please click on the button below to complete the Back to School Registration online forms to provide us with any updates to your information:
If after your child is enrolled and you move to a new address at any time during your child’s enrollment in the District, it is your responsibility to inform your child’s school principal of your new address.
Likewise, parent(s)/guardian(s) are responsible for informing school officials of any change(s) to the legal custody or guardianship of their child(ren).
In the event of any of the changes listed below, parent(s)/guardian(s) shall notify their child(ren)'s school or house principal (LHS only).
NOTE: Notification shall be made to the Department of Student Services at the Board of Education when individual school buildings are closed.
Provide updates for any of the following:
change of address
change of telephone number(s)
change of legal custody or guardianship
all current court orders from Domestic Relations, Juvenile, Probate or any other court that has exercised jurisdiction over the custody, guardianship or residence of the child/children as stipulated in O.R.C. 3313.672.

Use this site to update contact information such as phone numbers and email addresses for yourself or other contacts listed on your child(ren)'s record. Any address, guardianship, or custody changes should be communicated with your child's building principal or house principal (LHS only) and associated documentation provided.
Changes that require documentation:
Change of address - provide 2 proofs of residency
Current gas/electric bill-portion showing your name & address
Faxed confirmation of new utility service: (216) 228-8327
Fully executed lease: Includes landlord contact information and all persons living at this address
Owner Affidavit completed in full and notarized by landlord/property manager (only when proofs noted above cannot be provided).
Residency and Custody Affidavit completed in full and notarized by resident.
Change of custody/legal guardian - provide any and all current certified court orders from Domestic Relations, Juvenile, Probate or any other court which has exercised jurisdiction over the custody, guardianship or residence of the child(ren) as stipulated in ORC 3313.672.
